Monday through Thursday 8 am - 2 pm
Friday 8 am -12 noon

Cancellation and No Show Policy
If you are unable to keep your scheduled appointment, please call our office 24 hours in advance. Your phone call is critical in helping us provide continuous care to all of our valued patients.

Billing and Insurance
You are responsible for knowing your insurance limitation and policies. Please contact your insurance company to be assured that you have coverage for the services you will be receiving. Please be sure to bring your insurance card to EACH visit.

We will gladly file your medical charges with your insurance carrier; however, ultimately the payment is your responsibility. All insurance co-payments and deductibles are due at the time of service. If we do not have a contracted arrangement with your insurance plan, you will be expected to pay in full at the time of service. We accept cash, checks, MasterCard and Visa as payment options.

Prescriptions Refill
If you need a refill before you can be seen, CALL YOUR PHARMACIST, and request a refill. If there are refills remaining, ask the pharmacist for the next refill.

Please make this call at least 5 working days before you run out of medication if you use a local pharmacy, but at least 14 working days if you have a mail order source for your medications. Your pharmacist will send us a request by fax. Your chart will be reviewed and evaluated before the refill may be authorized.
